Leaking ceiling above the bay window in Redland

Job description:Water seems to collect on the flat surface above my bay window, (I am in the ground floor flat) and I think there are cracks in the top bit and water leaks through there and through my ceiling. I don't think it is a big job, however I have no idea what needs doing. I think I just need some one to check it out and fill in the cracks from above but I dont know how to. Sorry the decription is abit vague, I really need someone to have a look at it and see what the issue is
