Worcester combi - hot water not coming on properly and then not staying on in Heath

Job description:Worcester 240 combi - warter started going cold after a few minutes - on checking it, it seems it also now does not turn on the heat unless the flow is very high (hot tap full on only). Also there are black bits in the water - suspected to be from the boiler itself. Thermostat on the boiler has been faulty since moving it but not been a problem - This could be an item to be fixed too.
