Diagnose and fix damp problem in our kitchen in Freemantle

Job description:Victorian terrace house with kitchen to rear (includes area where old outside toilet used to be). There is damp on the inside of our end kitchen wall which appears to be coming up from the ground, as well as mould on the party wall behind the kitchen cupboards. The area outside the end kitchen wall is concreted and there is an old covered drain outside directly under the main damp area. We are keen to get the problem diagnosed and fixed as soon as possible.
