Large and growing crack in the ceiling of old Victorian flat. Will whole ceiling need replaced and can central rose and cornicing be conserved?

Probably and lathe and plaster ceiling unless it has been modernised. I assume there are people living above you so there will be constant foot traffic and probably heavy furniture etc. difficult to say without seeing it but a long term solution maybe to take down the ceiling and attempt to strengthen existing timbers (if poss) and maybe add some additional noggins. Whilst doing this you could also instal some mineral wool for better sound proofing and heat retension. Saving cornicing is very difficult, I would simply buy a reproduction replacement.
