Instant Hot Water system no longer providing constant hot water

Worcester Combi Gas Boiler fitted 18 months ago; heating works well but hot water cuts out after a couple of minutes showering. Remedy is to turn the water off and turn it back on, repeatedly, but pipes really bang too when the hot water runs out. Taps will run out of hot water too after a while - not what we expected from instant water heating system! What could the problem be?
