Repair of solid floor and replace tiles

When I moved into this house 3 years ago, there was impervious laminate flooring in the kitchen. I have now removed this and found the original quarry tiles, some of which were broken and repaired with concrete. I got the broken ones out and found a layer of screed underneath, about 8-10mm. The tiles were 15mm thick. There is a patch of concrete which was inserted to repair a hole in the floor, but you can see the hole is still there, about an inch across. I originally thought I would replace just the broken tiles with reclaimed tiles, but now I would like advice on what options I could consider. Is it possible or practical to put a layer of insulation in? There doesn't appear to be a damp course, how should this be remedied? Should I get rid of the rest of the old quarry tiles and put down new, thinner tiles? That room is the coldest in the house, and faces east.
