Lampshade fitting

Hi all, I have bought a ceiling lampshade to go in my new flat. It comes with a lampshade reducer which if used makes the circle too small and if I take it out the hole is too big and it just falls through. The diameter of the lampshade hole (without the reducer) is 40mm and I’d say the light fitting is about 2mm wider. Any ideas as to what I should add in to hold the shade up would be much appreciated. The problem seems to be the fitting and the wiring rather than the shade. It’s pretty old so may not be the usual standard size. All thoughts appreciated, thank you.

Have you removed the screw off bottom section of the lihgt fitting before slifding the light fitting on with the reducer attached and then screw it back on..
